Application forms can be obtained from the Institute on payment of Rs. 1,350/- (Rupees One Thousand Three Hundred and Fifty only) in person or by post by sending a Demand Draft drawn in favour of SDM Institute for Management Development, Mysore, payable at Mysore.

Take the printout of the same on A4 size 80 GSM paper (back to back) and fill-it up as per the instructions given in the application form.
You need to send the completely filled-in application form (first four pages only) along with a crossed DD of Rs. 1,350/- (Rupees One Thousand Three Hundered and Fifty only) drawn in favour of SDM Institute for Management Development, Mysore, payable at Mysore.
Leave the application number column blank. On receipt of your application, the Institute will allocate an application number and intimate the same to you. You should quote that application number for any future communication. The Institute will also send the Institute brochure along with the acknowledgement.
Send your filled-in application form, DD and all the documents as mentioned in the application form to "Admissions Office, SDM Institute for Management Development, No. 1, Chamundi Hill Road, Siddarthanagar Post, Mysore - 570 011, Karnataka" through reliable courier/registered post/speed post only and it should reach Admissions Office on or before the last date.
Please note that you need to send all in a big size cover and also mark "Application Form for PGDM 2009-11 Batch" on the top of the cover without fail. Do not keep more than one application form in the envelope. Use the envelope supplied if you have purchased the application form.
The candidate may note that his / her application would be summarily rejected if the application is incomplete in any respect, specifically if documents supporting qualifications are not provided, any mandatory field is left blank, the form is not signed, photograph is not attached, or if the candidate fails to meet the eligibility criteria. No corrections will be permitted after submission of the application form. However, in case a candidate identifies any mistakes in the submitted form, he / she may buy and submit a fresh form duly filled in.
Candidates buying the SDM-IMD application form from any coaching centres should submit the application form directly to the Institute.
SDM-IMD application forms once sold, will not be taken back and no claims of refund of application fee will be entertained.
SDM-IMD would not be held responsible for non-delivery or postal delays in receipt of completed SDM-IMD application form.
